TITLE: Health Information Management ClerkDEPARTMENT: Health Information ManagementFLSA: Non-ExemptSALARY: JOB SUMMARY:The HIM Clerk updates patient medical records, answers telephones, assists with medical office requests, and other customers regarding patient information. Must have the ability to multi-task and to work with all types of customers.QUALIFICATIONS:Education/Training: High school diploma or equivalent is required. Graduate of a Medical Secretary program is preferred.Experience: None required. Prefer one or more years of health information experience.License/Registration: None required.Population Served: N/AESSENTIAL FUNCTIONS:Planning and Prioritizing1. Demonstrates the ability to prioritize and multi-task throughout the day as needs arise, i.e., telephones, customers, and other requests.Information Gathering2. Files and delivers patient medical records to other departments and to physicians as requested.3. Is responsible for answering telephones with follow through on telephone requests, i.e., faxing.4. Assembles Emergency Department, Outpatient Laboratory, and Outpatient Radiology patient records.5. Is responsible for retrieval of archived patient medical records through various archival systems in the department, i.e., Master Patient Index Log or cards and outside storage.6. Is responsible for scanning in future scheduled surgery patients.7. Verifies daily assignment of new patient medical records numbers.Decision-Making8. Responds to all release of patient information requests, i.e., Patient Financial Services, Attorneys, and insurance companies.9. Assists with other administrative duties as requested.Communication10. Works with hospital departments as well as outside institutions to provide the highest standard of patient care possible through communication of needs and patient information required.11. Communicates with customers to handle personal requests and needs.Technical Proficiency12. Demonstrates the knowledge and skills required in understanding the laws governing health information.13. Follows the AHIMA Code of Ethics, adopted by the department.Teamwork14. Participates in Performance Appraisal process of colleagues when requested.15. Participates in solving team issues in a constructive manner.16. Attends and participates in office meetings.Quality17. Works with all customers in a positive atmosphere, so as to provide the best quality customer service as possible.18. Assists with quality improvement audits as requested.Outcomes19. Assembles daily Emergency Department, Outpatient Laboratory, Outpatient Radiology visits to be coded then scanned into the Electronic Data Management System.20. Files and retrieves patient medical records.21. Handles department requests for patient information.22. Prepares and scans archive medical records into the Electronic Data Management System.23. Demonstrates knowledge in HIPAA to provide confidential and secure patient health information.Personal Education24. Participates in ongoing educational activities as necessary to maintain competency in medical records, i.e., attending conferences/reading articles.Work Effectiveness25. Demonstrates the ability to work independently, prioritize, and to accomplish tasks in a timely manner.Reliability26. Demonstrates promptness in reporting to work and compliance with meal and break schedule.27. Completes tasks assigned to perform in a timely and efficient manner.28. Performs other non-essential functions as requested.PHYSICAL DEMANDS:Sitting: 80%. Standing/Walking: 20%.Occasional lifting/carrying of manuals, supplies, or medical records weighing up to 10 pounds.Occasional use of a step stool.Occasional stooping/kneeling/couching to retrieve supplies and medical records.Frequent climbing of stairs to reach other levels of the building.Frequent reaching/handling/fingering to use books, manuals, file, to write, to use computer and telephone.Frequent talking/hearing/seeing to communicate with other staff members and outside individuals/organizations.ENVIRONMENTAL CONDITIONS:Location: Inside.Frequent exposure to noise to cause distraction from computer printers, telephones, and outside generator.Minimal risk of injury due to proximity of moving parts on computer printer, copy machines, and mobile storage system.Minimal exposure to biohazardous materials.
