Description $5,000 hiring bonus for eligible external hires that meet required qualifications and conditions for payment. Physical Therapist (PT) - Physical Therapy at Providence Mount Carmel Hospital Under the general direction of the Rehabilitation (Rehab) Services Manager or Assistant Manager, the Physical Therapist assesses patient needs, develops patient treatment goals, and plans and implements the appropriate treatment program utilizing a variety of professional physical therapy procedures. Evaluates patient's progress and response to treatment and discusses with the appropriate physician as indicated. Maintains daily documentation of treatment, uses SOAP notes to indicate patient progress, and provides re-evaluations at least on a monthly basis. Follows treatment protocol established within the Rehab Services Department and adapts them to individual patient needs. Is familiar with the goals for the rotation to which assigned. This is a full-time, day shift position working 40 hours per week. Required Qualifications:

• Graduate from an accredited Physical Therapy program to become eligible for licensure
