Excelsior Wellness provides access to a comprehensive continuum of education, behavioral health, primary medical, social determinant, and human service providers, holistically rooted in the eight dimensions of wellness. Our subsidiaries include Excelsior Wellness Center, Excelsior Family Medicine, Excelsior Integrated Care Center, and Excelsior Holistic Schools. Excelsior Family Medicine is seeking a full-time, passionate, and energetic PMHNP to join our team. The successful applicant will have a daily appointment schedule and use their skills to identify and manage various healthcare issues in the outpatient setting. This position complements our interdisciplinary approach to whole-person wellness and will work with providers in primary care, behavioral health counseling, and psychiatric medication management. We serve a diverse population of youth and adults, most of whom have experienced trauma during critical developmental periods in their lives. Excelsior Family Medicine serves internally and externally referred individuals and families. Our organization is committed to high standards of care delivery and a patient-centered approach. Providers are supported by a team of professional staff and leaders experienced in clinical settings and healthcare administration. How to qualify: New Graduates will be considered for this position! Part-Time will also be considered! Active licensure as Advanced Registered Nurse Practitioner. Active prescribing authority. ANCC Board certification is required. 3 years of clinical experience preferred. Will consider new graduates! Ability to effectively work with and communicate with interdisciplinary teams, including behavioral health specialists, therapists, physicians, and other health care providers. Excellent organizational skills; communication skills; ability to process and analyze information effectively. The schedule is Mon-Fri, and may participate in call rotations. Core Responsibilities Include: Collects data from multiple sources using assessment techniques appropriate to enrolled individual's language, culture, and developmental stage, including but not limited to screening evaluations, psychiatric rating scales, genograms, and other standardized instruments to identify psychiatric/mental health conditions. Prescribes, monitors, and coordinates psychotropic and related medications based on clinical indicators of an individual's status, including results of diagnostic and lab tests as appropriate to treat symptoms of psychiatric conditions to improve mental health status. Ensures strict adherence to professional code of ethics in delivery of services. Analyzes the relationship between normal physiology and specific systems alterations associated with mental health problems, psychiatric conditions, and service delivery. Obtains and documents relevant health history, emphasizing mental health for individuals relevant to their specific needs. Differentiates between exacerbation and reoccurrence of a chronic condition and signs/symptoms of a new condition. Collaborates in a team setting with providers and leaders across disciplines within Excelsior and its affiliates. WHAT WE OFFER: Medical plans with a zero-cost option for you.
