Job Description:
Description The Care Manager RN is responsible for managing
patient populations with chronic illness and/or multiple
co-morbidities. This position works in coordination with multiple
PCPs and serves as the central link to an identified population of

Coursework/Training: Graduation from an approved, accredited school
of Nursing. - Upon hire: Washington Registered Nurse License - 2
years of work experience in an RN role. - Work experience revolving
around medications and IV solutions in terms of purpose,
administration, side effects and contraindications and administers
according to established standards and provider orders. -
Experience managing complex patients in an ambulatory setting.

position. The successful candidate's placement on the wage range
for this position will be determined based upon relevant job

pay range; additional compensation may be available for this role,
