Great work you’ll do here:

The Social Worker will provide medical social services to patients in the home through physician’s orders and under the supervision of the Agency Management. 


  • Assesses clients social and emotional needs related to illness and/or injury
  • Respond to treatment and adjustment to care including the relationship of the medical and nursing requirements to the home situation, financial resources and availability of community resources
  • Provides rehabilitative and supportive casework geared toward obtaining maximum benefits from medical care
  • Helps clients utilize family and community resources through referrals or linking directly with community health and welfare agencies
  • Works to achieve appropriate continuity of care to assure clients of ongoing services to meet their needs
  • Maintains ongoing outreach with community agencies in order to identify agency services and act as liaison between Agency and the community
  • Provides in-service education on social work related topics as needed/requested
  • Prepares clinical records (initial evaluations, plans of care, progress notes, discharge summaries) and submits them for the clinical record within agency policy
  • Recommends new policies and/or changes in current policies to improve quality of client services as well as efficiency and effectiveness of services
  • Provides care as outlined in the patient’s plan of care and documents appropriately in the record
  • Communicates with clients for visit schedules
  • Provides appropriate frequency of follow-up visits
  • Is knowledgeable of the agency’s QI goals and participates as directed
  • Participates in Professional Advisory Committee as requested
  • Understands and contributes to the financial management of cases
  • Completes other assignments and duties as requested and assigned to accomplish the goals and objectives of the Company

  • Capable of using professional judgement to delegate selected clinical tasks
  • Strong ability to be the public face of the organization, and serve as an advocate for Home Healthcare, Home Hospice and other service lines
  • Creative thinker who embraces thought leadership
  • Capable of making challenging decisions and willing to take responsibility for them.
  • Graduate from an accredited social work program with a Master’s degree in related field
  • One (1) year experience in a healthcare setting (hospital, clinic, rehabilitation center, etc) with qualified supervision
  • Experience should be in direct service to clients and their families experiencing social, emotional and/or medical problems.
  • Valid driver’s license and automobile insurance
